What issues can you help clients with?
I have several years of experience working with depression, anxiety, dissociative disorders, trauma, and autism spectrum. 

What does a typical appointment look like?
A typical appointment may take several different forms depending on the goals we have developed together. Generally, I work from a psychodynamic perspective that allows you to bring up the issues that you most want to work on each session, while also maintaining a focus on past experiences and patterns that may be contributing to the current issues. At times I may assign homework between sessions in order to continue to explore various aspects of therapy.

What insurance do you take?
I currently take Medicare, Beacon, and private pay on a sliding scale. 

What days of the week do you see clients? 
I am in the office Monday, Wednesday, Thursday and Friday from 12:00 until 7:00

If you are experiencing a psychiatric emergency please call Sonoma County Psychiatric Emergency Services at 1-800-746-8181 or go to your local emergency room.

How long will I need to continue to see you?
Change will sometimes be easy and swift but it can also be slow and even frustrating. During the course of therapy, Dr. Forman is likely to draw on various psychological approaches according, in part, to the particular challenges that you are working with and his assessment of what will best benefit you. Both of these factors may influence the length of treatment. Dr. Forman will regularly ask for your feedback and views on your therapy and its progress in order to assure that treatment is meeting your needs. 

What is your cancellation policy?
Making appointments is a vital part of the commitment to your own healing and therapy goals. It is also understandable that sometimes illness or other unavoidable circumstances arise that necessitate cancelling an appointment. My policy on cancellations is that I be given a minimum of 24 hours notice is required for rescheduling or canceling an appointment so that I am able to fill that time slot. Unless we reach a different payment agreement, a fee of $140.00 will be charged for sessions missed without such notification. This fee may be waived in the case of same day illness or if we are able to reschedule the appointment in the same week.
